Required Forms Before Ministry:
1. Form 1 - (required) Short Form (if not already filled out previously, unless it is a new issue(s) or I will specify if I need you to fill out the Long Form if I've not ministered to you before, or it has been more than 2 years.
Do not skip questions when filling out the form, or it won't go through.
2. Form 2 - Required Legal Binding Liability Agreement and form.
3. Form 3 - Bitterness Form—If you do not feel resentment, frustration, or unforgiveness or replay a person in what they did or doing today, you do not need to fill out this form. The biggest block to healing and bitterness. (Matt. 6:14-15).
