If you've been with me for ministry within a year, you don't need to fill out the short form unless something has changed. Please fill out the liability form.

When filling out the form, do not skip questions, or it won't go through.
2. Form 2 - (required) Liability form (if not already filled out).
3. Form 3 - Bitterness Form—If you do not feel resentment, frustration, or unforgiveness toward someone or something, you do not need to fill out this form. The biggest block to healing and bitterness. (Matt. 6:14-15)
